My other pick would be the movie Man Push Cart. I can't pick a single scene from it, and I don't want to give too much away. But this was the most emotionally devasting film that I've ever seen.

Possibly getting flamed for this but as a person who has lost a dog I was close to, Marley and Me takes the cake in regards to that kind of scene.

Oh and I'll also throw a really random one out there (it got me as a small kid): the ending in Harry and the Henderson's where he slaps him and basically has to treat him like crap so he'll go back to his own habitat.

A lot of the good ones have been mentioned, got to think outside the box to come up with something. Just browsed but I assume The Mist, Wicker Man (original) endings have been mentioned. You have the rape scenes in Last House on the Left and The Hills Have Eyes that are tough to watch for me personally.

great thread. for me the previously mentioned russian roulette scene in the deer hunter and the death of elijah wood's character in the ice storm are pretty intense.
i would like to add two scenes which i believe haven t been mentioned yet. the first is in all quiet on the western front (1930), where the main character kills a french soldier and then has to spend the night sitting next to him to avoid being hit by shells. his monologue (/dialogue with the corpse) about the pointlessness of war is rather powerful and to me one of the best anti-war moments in cinema.
the other emotional moment that comes to mind is the duel in kubrick's barry lyndon. such great suspense and the consequences barry must carry from his honesty are truly heartbreaking, leading to his demise

A couple good ones that haven't been mentioned:

One that definitely gets me is Jude when they come back to the apartment and the oldest child has killed the other children and hung himself ... although the end of the book was >>>> The end of the movie.

The scene in Jesus' Son where he comes back to his gf to find that she had commited suicide by od and he just sits there with the body for a while then eats the suicide note.

This film also has a bunch of them.

When Leonard sees his mother again after thirty years.

When the old man sees his wife again.

When Leonard gets but a few steps from the door to the outside world, then gets dragged away.

When Leonard tells Paula goodbye. She won't let him leave without a dance, and the moment sends him into unexpected stillness.

there's a few scenes in Born on the 4th of July that get me

when Kovic yells at his mom and they kick him out of the house ( http://www.youtube.com/watch?v=IUk8bJJg8Kg )

or the part where he has to tell the family he killed their son

the very end where his mom's voiceover is saying that she had a dream that he would be speaking to many people in an important way, and he's going out to speak at the DNC

Cruise might be a nut but he was legitimately awesome in that movie
