Hello how can I use this form.

I get 'you do not have permission to access this page...'

If you're trying to use the form and get the "you do not have permission to access this page..." error, post in this thread and I'll get you fixed up.

Once I fix it for you, I'll be deleting your post so you don't get spammed by others.

it is telling me:

Too many recipients - you are attempting to send to 8 users but you are only allowed to send to 2 users.

This shouldn't happen - are you using the form without editing? If you try to remove or change recipients, it could cause problems.
