My Hotmail is spamming everyone! Help!

I've never looked back since I swapped all my stuff over to GMail. I still see emails and msn chats from old contacts who have clearly been compromised - the sort of generic spam OP gave an example of.
What's the most common way to get exploited by this sort of thing? The people whos accounts are sending out spam seem too savvy to fall for a phishing attack, and I'd have thought that was the only way for someone to be able to hijack an account like that.
