It's a gutter or a small wrap, right? 4 outs twice to a straight that can still lose when paired at best. 9 outs to a wheel at worse. Only two backdoor flushes are possible as well since Hearts are covered.
A 3! on flop makes sense. If Villain caps, who cares as that doesn't really change our read on his hand. In fact, it likely narrows it to a 9 out scenario.
Even if he has 2345, its the same story really.
