I agree with your specific assessment, but opiates are difficult substances to properly manage, especially for long term problems.
MJ, while not specifically addressing pain very convincingly, has a wide range of ancillary benefits and poses very little risk so it is my default recommendation.
And yeah, acetaminophen is awful, as are most pharms.
