Are we talking live cash, or online? Live cash has a lot more "large" opening sizes. Same goes for 3betting. Online, it depends. Every situation has an "answer". For example, if your opponent in the BB is a loose player, what would be your sizing in order to steal the blinds in that spot? Most likely you want that sizing to be bigger because he will fold more often. Vice versa for people in the BB who are tight and not flatting many hands... you can get away with a min raise for the steal and effectively PRINT.
2) Again, dependent on a lot of factors but I was taught playing hands that flop well and can flop the nuts (89ss) and vill's want contain very many blockers to your hand. (Sorry doing this on a phone).
You won't see many 200b+ stacks at tables online, but it shouldn't change much if someone has 200bb vs 100bb. You shouldn't be making changes based on their stack unless they are short stacked... focus more on their tendencies and find a way to exploit that (mentioned above).
