Probably the most plausible origin of the term 86 comes from the New York State liquor code. Article 86 defines the circumstances for which a customer can be refused service.
The term is used in every bar in America. If you go into a bar and get too rowdy the bartender will say something to you like "You're 86'd, now get the **** out!!! You can be 86'd for a day, a week, a month, or forever.
One of the watering holes I drink at in Great Falls, Montana is called That Bar. On the wall behind the bar is an 86 list with over 400 names on it. The list goes back 20 years. The owners of the bar have security cameras everywhere in the bar and sit at home and watch what goes on in the bar. If an 86'd person comes in and the bartender doesn't recognize them the owners will call down and have the bartender remove the person.
