In the perfect world, changes in one piece of code should not cause unexpected side effects to other parts of the system. In a wisely designed/implemented application it doesn't happen very often.

In badly written software any insignificant change can easily blow the whole thing - in extreme cases to the point that it might be impossible or prohibitively expensive to change anything.
