Sign the form, [I forgot to ask to make sure it was sent in dollars but it was]
Give the guy $deposit + fee
He will give you a receipt with a MTCN Number on it, buy beer to celebrate, go home, type in MTCN number in the box on web-page and click submit.
BOOM load up poker site and play.
P.S. In case you have a problem: At a minimum, deposit on a site that responds to emails and/or PM's (i.e. not Carbon), Carbon gets ALOT of complaints (they are so bad for the industry). There are so many people that are unhappy with Carbon. Select a site that cares, that is attentive to your concerns (RPM, Hero, Everleaf (Minted), etc.). Read the posts, get a general consensus as to what the players are saying, then make an educated decision.
(If you did that, there is no way in the world you would select Carbon Poker, they are so bad.)
