No player in my country washes his hands during tournaments.
They are afraid of their hand being folded. Like when you cannot wait 'til the break.
Players and dealers picking their nose heavily before touching the chips and the cards you too are gonna touch.

That's why online is so difficult and live is so soft. Gotta pass up on hygiene that's the trade off

Headphones can obviously solve the first two problems, but runs the risk of missing other important information from the dealer or other players.

One time a guy at my table just said "Do you ever stop talking?" to a guy that never stopped talking... Even though it was awkward he talked less after that! Sometimes annoying players don't know they are being annoying, so you can either call them out, ride it out, or switch tables.
