I read in Super System2 that you should never cap the river without the nuts.
Does it still apply in a btn vs bigblind spot vs pat, pat, pat of villain who also saw me bluffing with blockers quite often?

I think it's fine to cap with #1 and #2.
Opponent may have weaker 7 lo or 85. They may even think you are bluff raising the river, I suppose? (especially considering your recent actions, like you said).

Four bets? We're talking about four bets? Absolutely put in four bets. More than five you need a special reason, but four bets of course.

If the BB check-raises, sure.

No, I mean donk/3-bet from OOP as opposed to check-raising.

if no cap, would raise/call be standard?

I would call the 5-bet and expect to see a wheel often.

pre-pat = always #16 or worse.

But hitting a 4 on second and a 3 on third after AQ and T usually means opponent has 235 queen +X. If I did not have ADD I would call a 3bet on river. Good discarding 8.
