It's certainly possible. Insofar as I can tell, it's from (or based on) an Answers in Genesis science curriculum for Christian schools and home schoolers. And here's a newsletter from a private school that had the author come and speak to their students about dinosaurs and Genesis, so it being real isn't outside the realm of possibility. Then again, the fact that is says "4th grade science quiz" makes me quite incredulous that this particular example is real, even if similar real examples might be available.

Internet 101: assume any image you see is fabricated as a joke until presented with copious information to the contrary.

E.g. I was suspicious of the Fum & Gebra video until National Geographic covered them as a story. I mean, the video looked authentic but who can be sure?

No idea if that particular photo is real, but as part of my college education I observed a wide variety of school settings, and one of them was a Christian elementary school where the kids were being taught similar material. I assume the kids were tested on such material, and if they were, the quiz would likely look somewhat similar.

Even then (when I was still religious) watching it happen made me quite uncomfortable.

I also saw "Christian-ified" material when I observed over the course of a few months at a Christian middle school.

I actually took a college biology course at my Christian college where the entire content was young Earth/irreducible complexity.

This is actually part of the Ken Ham/AiG "curriculum" though. Seriously, that is their recommended snappy comeback to science-minded types. Biblical fundies: from religious certainty to radical skepticism in zero seconds flat.
