1) Real name, age , location, Skype & Screen names @ all sites that you play on.
2) Why you want/need a stake.
3) Why you feel you would be a good choice.
4) What games and limits that you play (number of tables, use of HEM, Table Ninja etc) and are looking to be staked for.
5) Poker History and any relevant information so we can get a feel for who you are, where you are coming from and going to in your poker journey. (Include any coaching/training site memberships etc)
6) References - If you don't have any, say so, please don't make stuff up ... any lying or falsehoods that come to light will be immediate grounds for termination of contract.
