Yes, they can and usually do send out bills for any unpaid services. I don't know the situation as you left, but I'm guessing there was confusion over what you were paying, or there were delayed charges that weren't included at the time. Often physicians bill separately from the hospital, too. Yes, you are supposed to pay it. If you don't pay it they can and often do take you to court. If you show up, you can make a case as to why you shouldn't pay, but "I don't want to" won't get you far. If you don't show up, the provider will get a summary judgement against you. And then they can get more help from the courts to tap your paycheck directly from your employer if you still don't pay.
Medical debt is the #1 cause of bankruptcy in the US (say the headlines). The medical industry is absolutely cut-throat about it. If you have three hours, read:

If you're from a far off land, I'm not sure there's a lot they can do to force payment. But IANAL. I have no idea how a civil judgment rendered by a US court would impact someone from Canada or Australia or wherever.
